• 4 passport sized personal photos - with name at the back of the photo
• 2 copies of NRIC
• 1 copies of parent's NRIC (for PTPTN applicant only)
• A copy of guardian's latest pay-slip or confirmation of salary letter/ Pension Documents/ Pension's Indentification Card (for PTPTN applicants only)
• A copy of the CIMB statement Accounts contain with the account no and name (for PTPTN only)
• A copy of SSPN payment slip and BSN slip (PTPTN) with an amount of RM 5

• 2 copies of passport with full pages. Validity of passport MUST BE more than 18 months
• 2 copies of O level/ equivalent (All Applicants)
• 2 copies of A level/ equivalent (DEGREE applicants)
• 6 passport sized personal photos with white background (35mm x 45mm)
• 1 copy of scratch card & Yellow card (African country)/ 1 copy of No objection Certificate (Sudan only) / copy of letter of eligibility (LOE) (Iranian only)
• 1 copy of IELTS/ TOEFL or equivalent
• Pre-medical under EMGS registered panel clinic at home country
